Hi patient_first_name,

Your lab results are now available and can be viewed securely through your patient portal. Here's how to access them and what to expect.

• Access Your Results: portal_url

To view your results, log in to the patient portal using your existing credentials. If you haven't created an account yet, the portal homepage has a simple registration process — you'll just need your date of birth and the email address on file with us.

UNDERSTANDING YOUR RESULTS

Lab results are displayed alongside reference ranges that indicate whether your values fall within normal parameters. However, interpreting results in context — considering your medical history, medications, and symptoms — requires clinical judgment. Please do not make any medical decisions based solely on what you read in the portal.

NEXT STEPS

• If provider_name has identified values that require attention, our office will contact you directly to discuss follow-up

• If your results appear normal and you don't hear from us within 3–5 business days, you can assume no immediate action is needed — but feel free to call if you have questions

• If you have concerns about your results before you can speak with the provider, please call our office at me::phone
